Database is down / Provider cannot be found

Error Message:

"Provider cannot be found. It may not be properly installed."

Solution:

This was the cause in at least one occurence we've seen: The file MSDATL3.DLL could not be found because the system path didn't
include "C:\Program Files\Common\System\Ole DB".

In addition, the FogBugz user needs to be given full permissions on this file.
